PICO vs. PICOS: Building a Searchable Review Question

Turning a general research interest into a genuinely searchable, screenable systematic review question is one of the most consequential early steps in the entire process, and PICO and PICOS are the two most widely used frameworks for doing it. Understanding the difference between them, and how to actually apply either one, prevents a surprisingly common source of downstream problems.

What PICO stands for and does

PICO breaks a question into Population, Intervention, Comparator, and Outcome. "In adults with type 2 diabetes (P), does metformin (I), compared to lifestyle intervention alone (C), improve HbA1c at 12 months (O)?" This structure forces the specificity a search strategy and eligibility criteria both genuinely need -- a vague question like "what's the evidence on diabetes treatment" cannot be searched or screened consistently, while a properly framed PICO question can.

What the added S in PICOS covers

PICOS adds Study design as an explicit fifth component, specifying which study types are eligible for inclusion -- randomized controlled trials only, or a broader set including observational designs. This might seem like a minor addition, but it resolves a genuinely common source of eligibility ambiguity: without an explicit study design specification, screeners can reasonably disagree about whether a well-conducted observational study should be included alongside RCT evidence, particularly in areas where randomized evidence is limited.

Why the study design component matters more than it looks

Your risk-of-bias tool choice, your GRADE starting point, and often your synthesis approach all depend directly on which study designs you've decided are eligible. Specifying this in your framing, rather than deciding it informally during screening, connects your question framing directly to methodological decisions you'll need to make consistently throughout the rest of the review.

A practical process for building your question

Start with your general topic and ask what specific population you're actually interested in -- age range, disease severity, setting, or other characteristics that meaningfully narrow the question. Then specify the intervention precisely enough that a reader could judge whether a given study's intervention counts as the same thing, rather than a related but different one. Choose your comparator deliberately -- placebo, standard care, an alternative active intervention -- since this shapes what your effect estimate actually means. Define your outcome with the same precision, including the specific measurement and timepoint if relevant. Finally, if using PICOS, decide and state your eligible study designs.

Testing whether your question is actually searchable

A useful check: try translating your framed question directly into search terms for one database. If you can identify clear, specific search terms for each PICO or PICOS component, your question is specific enough to build a real search strategy from. If you find yourself needing to use very broad terms because your question components are still vague, that's a signal to narrow the framing further before moving on.

When PICO is sufficient and when PICOS adds real value

For reviews where study design eligibility is uncontroversial or already effectively fixed by the topic area -- a review restricted to RCT evidence in a field where RCTs are standard, for instance -- PICO alone may capture everything needed. For reviews where the eligible study design mix is a genuine, non-obvious decision, particularly reviews including both randomized and observational evidence, PICOS's explicit study design component is worth the small extra effort of stating it clearly.

Beyond intervention reviews

Both frameworks were developed primarily for intervention effectiveness questions, and other review types use adapted versions -- PECO substituting Exposure for Intervention in reviews of harms or exposures rather than treatments, or PCC (Population, Concept, Context) for scoping reviews with a broader mapping purpose rather than a narrow effectiveness question. Choosing the framework that actually matches your review's purpose, rather than forcing PICO onto a question it doesn't fit well, produces a more genuinely useful and searchable question.

It's common, and legitimate, to discover during an initial scoping search that your framed question needs adjustment -- a population that turns out to have too little published evidence to review meaningfully, or an outcome measured so inconsistently across studies that your original framing needs narrowing. Making this kind of adjustment before finalizing your protocol and beginning formal screening is normal; making it after screening has already begun, without documenting and justifying the change, is the kind of undisclosed deviation that draws legitimate reviewer scrutiny later.

A collaborative framing process works better than a solo one

Building your PICO or PICOS question in discussion with at least one other person familiar with the topic area, rather than alone, tends to surface ambiguities faster -- a co-author or methodology consultant will often ask exactly the clarifying question a database search would later expose the hard way, catching a vague component before it costs you a week of imprecise search results. This collaborative framing step is a small time investment that regularly pays for itself many times over once search and screening begin.

PICO and your eventual GRADE assessment

The outcome component of your PICO or PICOS framing connects directly to your later GRADE certainty ratings, since GRADE assesses certainty separately for each outcome specified in your review question. A vaguely defined outcome at the framing stage creates ambiguity that resurfaces at the GRADE stage, when you need to state clearly and specifically which outcome each certainty rating actually refers to -- another concrete reason precise framing early pays off well beyond just the search and screening stages. A few extra minutes spent sharpening your outcome definition at the framing stage routinely saves considerably more time at every stage that follows, all the way through to your final GRADE assessment. This is one of the clearer illustrations of how early-stage methodological choices ripple forward through an entire systematic review -- a decision made in an afternoon of question-framing can meaningfully shape how much rework is needed months later, when your review reaches its final synthesis and certainty-rating stages. Reviewers who see a clearly, precisely framed question at the very start of a manuscript tend to extend more confidence to everything that follows, since precise framing signals the same care was likely applied consistently throughout the rest of the methodology.
